The Muttizettel — also called Mutterzettel or Erziehungsbeauftragung — is not an official form. German youth protection law lets 16- and 17-year-olds stay at public dance events without a companion until midnight. After that only with a parent or a person they have put in charge, who must be able to prove it on request — that's what the Muttizettel is for.
Any adult your parents have put in charge who is able to look after you — so not drunk, for example. They stay close the whole time and leave the party with you. Some clubs ask for an older companion or let one adult accompany only one or two teenagers.
The law doesn't explicitly require it, but most clubs use it to check the signature. The copy must be recognisable as a copy, and you may black out the access and serial numbers.
Yes. Organizers decide who gets in under their house rules, and many parties are 18+ anyway. Check the event's age limit before buying a ticket.
Better not. Clubs usually accept only a note with that night's date and event. Make a new one for each party — it takes two minutes here.
Under 16 the law only allows dance events with a parent or an appointed companion, at any time of night. In practice most clubs only admit guests from 16.
The Muttizettel doesn't change the alcohol age limits: beer, wine and sparkling wine from 16, spirits and anything containing them from 18. Smoking is allowed from 18.
